New Patient Information
As new patients to Primary Health and Wellness Centers, you will be asked to provide personal information and insurance information. Please be prepared to provide this information when you schedule your appointment. If you are already an established patient and your information has changed, please let our staff know when you arrive to allow us to update your information.
For your convenience, we have attached the forms you will be asked to complete and sign when you arrive for your first appointment. You may print these forms and complete them in advance if you would like. Doing so will save time and paperwork for you on the day of our appointment. Just click on the document link below to print the required information. If you prefer to complete them in the office, we ask that you arrive 30 minutes early for your appointment. If you have any questions as you are completing the paperwork, please call either of our offices at 724-832-7877 for the Greensburg office and 724-864-9595 for the Irwin office.
During your new patient visit please bring your insurance card, valid ID, medication list, and any copay if required by your insurance company.
If you are a current patient and would like to request a copy of your medical records for your own files to to be sent to another physician's office. Please compete the docoment below by clicking on the file, complete all of the necessary information, and bring it to the office for the staff to process your request.
